Home health care consists of a variety of health care services that can be provided in your home for an illness or injury. Home health care services are just as effective as care you get in a hospital. Additionally, getting in-home care is safer and much more convenient. After your doctor refers you for home health services, the home health agency will contact you to schedule an appointment at your home and assess your health. Throughout the process, agency staff will stay in touch with your doctor to keep them updated about your care plan and progress.
Ideal Choice Home Health is licensed by the California Department of Public Health (CDPH) and accredited by the Accreditation Commission for Health Care (ACHC). We are located in Sherman Oaks, California and provide service in the following counties: Los Angeles County, Riverside County, San Bernardino, Orange County, and Ventura County.

In addition, our client’s have access to Medical Social Workers, whom assist with social and emotional factors related to their health as well as inform them about community resources.
